What is a colposcopy, and why might you have been referred?
A colposcopy is a detailed examination of the cervix using magnification. It is commonly recommended when cervical screening or HPV testing identifies findings that need closer assessment.
An abnormal smear or positive high-risk HPV result does not automatically mean that you need treatment, and it does not mean that you have cervical cancer. Colposcopy helps your consultant understand what the screening result actually means for you.
Some patients need reassurance or surveillance only. Others may require a targeted cervical biopsy to determine whether abnormal cells are present. If higher-grade changes are identified, treatment such as LLETZ may then be considered where clinically appropriate.
The purpose is clarity, not simply another test.

Common questions following an abnormal cervical screening result, high-risk HPV result or referral for closer cervical assessment.
Do I need a colposcopy if I have HPV?
Not always. A positive high-risk HPV result does not automatically mean that colposcopy is required. Some patients are advised to have repeat cervical screening, while colposcopy is more commonly recommended when HPV is accompanied by abnormal cervical cell changes, persists over time or there is another clinical reason for closer assessment.
Does an abnormal smear mean I have cervical cancer?
No. An abnormal cervical screening result does not mean that you have cervical cancer. It usually means that HPV or cervical cell changes have been identified and need further assessment. Colposcopy helps determine whether reassurance, surveillance, biopsy or treatment is appropriate.
What happens during a colposcopy?
A speculum is used so the cervix can be seen clearly. Your consultant examines the cervix under magnification using a colposcope and applies special solutions that help highlight areas of abnormal cells. If an area needs histological assessment, a small targeted cervical biopsy may be taken.
Is colposcopy painful?
Colposcopy is usually well tolerated. Some patients experience discomfort from the speculum or mild stinging when the solutions are applied. If a biopsy is required, there may be brief pinching or cramping. Your consultant will explain each step during the examination.
Will I definitely need a cervical biopsy?
No. A biopsy is only taken when your consultant identifies an area that requires tissue assessment. Some patients can be reassured or followed with surveillance without needing a biopsy.
What do CIN1, CIN2 and CIN3 mean?
CIN means cervical intraepithelial neoplasia and describes abnormal cervical cell changes rather than cervical cancer. CIN1 is low grade, CIN2 represents more significant changes and CIN3 represents high-grade changes. Management depends on the grade, your previous results and your individual clinical circumstances.
Can treatment be performed during the same colposcopy appointment?
In selected patients, treatment may be possible during the same visit where it is clinically appropriate. In other cases, a biopsy or previous results need to be reviewed before treatment is planned. Your consultant will explain which approach is appropriate for you.
What is LLETZ or LEEP?
LLETZ, also known as LEEP, is a common treatment used to remove an area of abnormal cervical cells. The removed tissue is sent for histological examination, and follow-up is planned according to the result.
